What Is Concierge Medicine and Is It Worth It?

September 4, 2026

What if your healthcare relationship started with knowing you, rather than waiting for something to go wrong?

For many people, healthcare has become something they navigate when they have a problem. An appointment is made, a concern is discussed, a prescription or referral may follow, and life moves on. But what happens when you want something different - a physician who knows your history, understands your priorities and remains involved as your health changes?

That is part of the idea behind concierge medicine.

Also called membership medicine in some settings, concierge medicine is a healthcare model in which patients pay a recurring membership fee for an ongoing relationship with a physician or medical practice. Depending on the practice, that membership may provide greater access to the physician, longer appointments, virtual care, preventive services, chronic disease management and care coordination. The exact services, fees and structure vary considerably, so understanding what a particular program actually provides is important.

For people considering a concierge medical plan, the question is not simply whether concierge medicine is "better." A more useful question is whether this type of physician relationship fits the way you want to approach your health.

What is concierge medicine?

Concierge medicine is a membership-based approach to primary and personalized medical care.

Instead of relying exclusively on traditional fee-for-service appointments, a patient pays a recurring fee to participate in a medical practice. The membership may provide services that are included within the fee, while other healthcare expenses, such as medications, specialist visits, hospital care or certain testing, may remain separate.

The American Academy of Family Physicians describes direct primary care as a model in which patients pay a physician or practice directly through periodic payments for a defined range of primary care services. These models can include enhanced communication, extended visits and personalized, coordinated care.

Concierge medicine can be structured similarly, although individual practices use different models.

Cleveland Clinic, for example, describes concierge medicine as a paid membership service that can provide direct access to a dedicated primary care physician, preventive care, chronic disease management, virtual visits and coordination with specialists.

The important word is relationship.

A concierge practice is designed around an ongoing connection between the patient and physician rather than treating every appointment as an isolated event.

Why does having a continuing physician relationship matter?

Healthcare becomes more complicated when your doctor only sees individual pieces of the picture.

A change in blood pressure may mean something different depending on your medications, family history, sleep, stress, activity level and previous readings. A new symptom may be easier to understand when your physician already knows your medical history. A laboratory result may be more meaningful when considered alongside previous results rather than viewed as a single number.

Continuity does not guarantee a particular medical outcome, but it can give a physician more context when making clinical decisions.

It can also make healthcare feel more connected.

Instead of asking, "Which doctor do I call?" or trying to remember which office has your latest information, patients in a concierge model may have a consistent physician or care team who knows their history and can help coordinate the next step.

Cleveland Clinic describes this relationship as one of the central features of concierge medicine, noting that membership programs can allow physicians to spend more time getting to know their patients and their health goals.

What can a concierge medical plan include?

There is no universal concierge medicine package.

One practice may emphasize primary care and virtual access. Another may focus on executive health, preventive testing or chronic disease management. Some programs include extensive health assessments, while others concentrate on direct physician access and routine medical care.

Common services can include:

  • Primary care
  • Preventive care
  • Chronic disease management
  • Virtual physician visits
  • Direct communication with the medical team
  • Lifestyle and health counseling
  • Care coordination
  • Health monitoring
  • Specialist referrals when appropriate

The important consideration is to look beyond the phrase "concierge medicine" and understand exactly what is included.

Patients should also determine what remains outside the membership. Hospitalizations, emergency services, medications, specialist care and some laboratory or imaging services may have separate costs depending on the program. Cleveland Clinic, for example, explains that its concierge membership fee does not generally include specialist appointments, hospital stays or emergency services.

Is concierge medicine only for wealthy people?

Concierge medicine has traditionally been associated with higher-income patients, but the membership landscape is broader than a single stereotype.

Different practices use different pricing models and levels of service. Some focus on accessible direct primary care, while others offer highly comprehensive executive or longevity programs.

That means the more useful question is not whether concierge medicine is "luxury healthcare." It is what you are paying for and whether those services have value for your circumstances.

Someone may value convenient physician access because of a demanding schedule. Another person may place more importance on preventive care and continuity. A family may want a single medical relationship that follows the household over time. Another patient may be interested in a more comprehensive health-monitoring program.

The reasons can be very different.

How Bespoke Concierge MD approaches personalized care

Bespoke Concierge MD takes a tiered approach to concierge healthcare, with plans designed around different levels of physician access and health priorities.

The current U.S. plan structure includes DPC Single, DPC Family, Optimal Performance and Executive Health, allowing patients to select a level of care based on their individual needs.

The DPC Single membership is centered on an ongoing personal physician relationship, with virtual care, physician visits, virtual urgent care, lifestyle medicine and chronic disease management.

DPC Family extends the concept to households, providing care for two adults and children, along with pediatric advice, sick visits, virtual urgent care and broader family access.

For patients who want to explore health and performance in greater depth, the Optimal Performance membership adds expanded biomarker testing and services focused on areas such as hormone and performance optimization.

Executive Health represents a more comprehensive level of personalized care, with the current program including advanced biomarkers, genetic testing, epigenetic age testing, advanced cancer screening and other specialized services.

The purpose of having different levels is not to suggest that everyone needs the most comprehensive option.

It is to recognize that healthcare priorities are different.

What about insurance?

This is one of the most important questions to ask before joining any concierge medical plan.

A membership fee and health insurance are not necessarily the same thing.

Many concierge programs operate outside traditional insurance arrangements for some or all physician services. Patients may still maintain health insurance for hospital care, specialists, prescriptions, laboratory services, imaging and other covered healthcare.

Medicare specifically states that concierge membership fees generally are not covered by Medicare, although Medicare-covered healthcare may still be available depending on the arrangement.

Before joining a program, ask what the membership includes, what is billed separately and how the practice works alongside your existing insurance.

Who might consider concierge medicine?

Concierge medicine may appeal to people who want a more continuous relationship with their physician.

That can include adults who want more convenient primary care, families looking for connected medical support, professionals with demanding schedules, frequent travelers, people managing chronic conditions or individuals who want to take a more proactive approach to their health.

It can also appeal to someone who simply wants to know, Who is my doctor, and how do I reach them when I need them?

That is a very different question from searching for a doctor only when something goes wrong.

So, is concierge medicine worth it?

There is no universal answer.

The value depends on what you want from your healthcare relationship, what services you actually use, the membership cost and what remains outside the membership.

For some people, the primary value may be access and convenience. For others, it may be continuity, preventive care, care coordination or having more time to discuss health concerns with a physician.

The most important step is to understand the model before making a decision.

Ask who your physician will be. Ask how communication works. Ask what is included. Ask what costs extra. Ask how emergencies and specialist care are handled. Ask how the practice works with your existing insurance. And, most importantly, ask whether the physician relationship matches the kind of healthcare experience you want.

At Bespoke Concierge MD, the approach begins with that relationship. Its nationwide network is designed around direct access to physicians and different levels of personalized care, from primary care and family medicine through performance-focused and longevity-oriented programs.

Because healthcare is personal, the right plan should start with the person - not simply the package.
